PRE-APPOINTMENT CHECKLIST

 

WHAT TO BRING TO YOUR APPOINTMENT:

If you are a new patient or if has been over a year since your last appointment with Premier Prosthetics, pre-registration is recommended. You will be asked to provide insurance and medical history information. Specific insurance information we will need includes:

1. Current insurance card and State ID
2. Physician’s prescription and referral (when required) for medical services
3. If a Workers Compensation case, please bring case managers contact information, workers comp number and DOI.
4. If a Medicaid case, please bring your current Medicaid card.
